These are common questions about provider network participation and the referral program. Please do not hesitate to contact with any additional questions about EZ’s network, program and processes.
There is no fee for participation and all licensed dentists are eligible to participate. For more information, please contact the provider relations department at EZ Dental.
No, by signing an EZ Dental Provider Agreement you are agreeing to accept referrals from EZ Dental. You will be contacted directly by an EZ Dental representative when a case presents itself in your area. If a Workers’ Comp carrier contacts you directly please complete the Provider Referral Form or call 1-855-9EZ-COMP.
When a Workers’ Comp referral is received we obtain all of the patient’s demographic information and injury information. The file is then reviewed to determine which provider type is best suited for the patient. If there are multiple providers in the area then the patient will be sent to the office that is closest to their home or work.
All claims are reimbursed 30 days after receipt of a clean claim or such time as set forth by law and in accordance with Provider Reimbursement Schedule on Exhibit A. A clean claim will include proper coding, supporting chart notes, and signed authorization form.
EZ Dental will provide you with written authorization for the initial evaluation. After the initial evaluation you will send in your treatment recommendations, after review we will send you another authorization for treatment.
